Welcome to Val Verde Dental! We are excited to meet you at your scheduled appointment.

Please arrive 15 minutes early to fill out new patient forms or print the forms out and bring it with you. We ask that you bring your dental insurance card to the first appointment also.
We want you to feel absolutely comfortable with the procedures and services you receive at Val Verde Dental.
For your convenience you may print and fill out the "Patient Information" and "Medical History" forms to bring with you on your first visit.
The forms are in Adobe (PDF) format.
